A company is looking for a Telephonic RN Nurse Care Manager.
Key Responsibilities
Coordinates care services with the pod leader to ensure
comprehensive patient access to healthcare
Develops and manages personalized care plans, addressing care gaps
and ensuring access to resources
Performs thorough nursing assessments via telephone and provides
education on disease management and prevention
Required Qualifications, Training, and Education
Active Registered Nurse License
2+ years of care management experience in community, health plan,
or hospital systems
Strong clinical skills and proactive thinking
Knowledge of Medicare regulations and home care standards
Bachelor of Science in nursing or related field preferred
