You Do in this Role:The Licensed Clinical Social Worker will
provide a full range of social work services in collaboration with
other CSMC staff to meet the psycho-social assessments, crisis
intervention, Care Coordination, transitions management and
discharge planning needs of patients and families. Successful
candidate will provide, amongst other primary social work
functions, supportive counseling, community resources, and
coordination of family conferences.
Requirements:Education: Master of Social Work (MSW)
required.License: Licensed Clinical Social Worker (LCSW)
required.Experience: A minimum of 3years' Acute Care Hospital
experience required.Demonstrated ability to work with a
multidisciplinary team.Proficient in the use of Microsoft Office
Suite.
