Job Description:

MindPath Care Centers, a division of Community Psychiatry, is seeking a dedicated Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team of expert mental health professionals. This role involves providing comprehensive psychiatric care, including assessment, diagnosis, and treatment of mental health conditions through both therapy and medication management. As a PMHNP at MindPath, you will be pivotal in fostering wellness and enhancing the mental health of our diverse patient population. Conduct psychiatric evaluations and assessments to diagnose mental health conditions. Develop treatment plans that include both pharmacologic and non-pharmacologic interventions. Prescribe medications and monitor their effectiveness, adjusting plans as needed. Provide crisis intervention services as needed. Collaborate with multidisciplinary team members including psychiatrists, psychologists, and social workers to coordinate care. Educate patients and families on mental health conditions and management strategies. Maintain patient confidentiality and document care in electronic medical health records accurately. Strong clinical skills in psychiatric assessment and medication management. Excellent communication and interpersonal skills, adept at patient education. Ability to work independently as well as part of a diverse clinical team. Proficient in the use of electronic health records. Board Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC). DEA registration.
